WOES Tech Support Request Form


This form is to be completed by the person having the technical issue.  Please do the following before you submit this form. 

  1. Reboot the computer and check to see if the problem resolved itself.
  2. If problem persists and the computer is unusable, place a "Do Not Use" sign on the computer
  3. Make sure all files are saved  to network folder in case tech needs to reboot or re-image your computer.

Enter your name.

First Name
Last Name  

Enter your room number.

Description of the problem. Be specific.

 

Enter your computer's asset number (if you can't find the asset number, enter the serial number).


Enter your IP number (usually hand written on the computer).


When does this happen?

Please understand that if it is a Groupwise issue and your Groupwise password is not your regular Novell password, the tech will reset it back to the Novell password in order to complete your request.

Some requests will be handled remotely (from outside the school).  The tech will send you a message via Novell messaging a few minutes before logging into your computer.  Please make sure you do not leave any files unsaved or they could be lost.

Please print your confirmation form for your records.
