I, ________________________, parent/guardian of __________________________have read the School System’s Student Internet Safety and Responsible Use Agreement.

I hereby release the school system, its personnel, and any institutions with which it is affiliated, from any and all claims and damages of any nature arising from my child's use of, or inability to use, the school system’s computer system, including, but not limited to claims that may arise from the unauthorized use of the computer system to purchase products or services or exposure to potentially harmful or inappropriate material or people. I understand that I can be held liable for damages caused by my child's intentional misuse of the computer system.

I will instruct my child regarding any restrictions against accessing material that are in addition to the restrictions set forth in the School System Policy and Regulations. I will emphasize to my child the importance of following the rules for personal safety.

I give permission for my student to use the Internet. I understand that this permission includes permission for my student to access information through the Web, receive an individual e-mail account, engage in other educationally relevant electronic communication activities, and provide personal information to others for education or career development reasons or as approved by school system employees.

 

 

I give permission for the school to post the following information/material on the Internet:
1. Images of student using limited student identification  (first name and last initial or other school-developed identifier). 
2. Student work, recognition, class rolls and team rosters may be posted with the limited student identification. All student-posted work will contain the student's copyright notice using the limited student identification.

I give permission for the school to post my student’s photo, work, class rolls and team rosters-- accompanied by full name-- on the Internet.

I understand that I may revoke any of these permissions, at any time, by submitting a new permission form to the school.  I further understand that the Family Educational Rights and Privacy Act, 20 U.S.C. § 1232g, prohibits the disclosure of any education record regarding my child without my written consent.
