Eligibility

Effective the 2008-2009 school year, a parent or guardian of every student athlete will be required to attend a Rules and Responsibilities meeting.  Student athletes will not be eligible until a parent or guardian has attended this meeting.  Please check with your child's coach for time and dates.

• Athletic Physical
• Informed Consent
  Note:  Both forms are required every 12 months
• Permission form for alternative transportation
  Permission to ride home with someone other than a parent/guardian
