NEW IMMUNIZATION REQUIREMENT FOR 6TH
GRADE ENTRY
Dear parents of 5th grade
students.
All 6th graders are required to have a booster dose of Tdap
upon entry into the 6th grade. This protects against tetanus, diphtheria and pertussis
and can be given anytime after they turn 10 years of age. You
will be asked to bring proof of vaccination to the school on the
first day of 6th grade. Acceptable proof of vaccination is a copy
of the child's Certificate of Immunization from a health care
provider. If proof is not
provided a child could be suspended until
proof of immunization is provided.
You can get
this vaccine at your doctor's office or local health department.
There is no charge for
the vaccine because it is required by law although your health care provider might
charge for administering the shot. When your child gets the vaccine, please
send a copy of the
documentation so their records can be updated. The doctor nor
the Health Department will send proof of this vaccine to the school
so please request documentation.
