   • Green Physicians Authorization

   • Pink Over-the-counter Medication

   • Student Information form for Chaperones

   • Medical Information

   • Terms and Conditions of Participation

   • Student Responsibilities

   • Student Budget (Sent to you as an attachment or on UK website)

   • Copy of Shot Record

   • Copy of Passport

   • Copy of Procedure for lost or stolen card

2. Latest Tentative Itinerary (copy for checked baggage and carryon) This will include important  phone numbers

4. ID Photos - Mr. Mac

5. Copy of UK Check list  

6. Homesickness, Jet Lag, and Getting Along

10. Keeping your Money Safe While Abroad

11. Packing

    • Personal item = purse, briefcase, laptop

    • Carryon = less than 51 inches

    • Checked bag = 

      • less than 62 inches and 

      • less than 50 pounds

12. Money and Budget

    Baggage
    

    • Put your name on the outside and inside of your bags. 
    • Put a copy of your itinerary in each checked bag.
    • Pack valuables in your carry-on bags not in your checked bag.
    • Contact information, and other documents necessary to your travel should not be in your checked baggage. (This will be given to you at our meeting)
    • Itemize. It sounds tedious, but when an airline asks what was in your bag, you don't want to forget anything of value.
    • Make sure the correct destination tag is on every bag.  Get a claim ticket for each.
